WP Video Polls is a plugin for WordPress that makes it easy and fast to create interactive polls for interaction with visitors to your site.

In addition to just collecting information, you can monetize the answers and turn the polls into sales funnels right on top of the video.
With the Premium Pro version, you can create different actions depending on the response.
Based on different answers, you can add conditions to where you want to send the persons after having finished the poll.
You can also set rules to jump to specific questions depending on the reply of the previous one.
Other actions can be downloading a gift, which is an excellent incentive for getting visitors to finalize the poll. You can even set conditions for different download gifts based on their responses.
